Ponies as Partners is currently accepting new clients.  Scheduling is flexible.  Please call Sabina Silvia at 401-683-9437 or e-mail us at Sabina@poniesaspartners.org

While not required for the first visit, the following needs to be completed and returned prior to final acceptance into our program.

1. Participant’s Application and Health History
2. Authorization for Emergency Medical Treatment Form
3. Participant’s Medical History & Physician’s Statement
